Flexible PRN Schedule | In-House Rehabilitation Team
Wyomissing Health & Rehabilitation Center is seeking a compassionate and motivated PRN Physical Therapist Assistant (PTA) to join our in-house rehabilitation team in Reading, Pennsylvania. This is an excellent opportunity for a PTA looking for flexible PRN hours while making a meaningful impact on the lives of residents in a Skilled Nursing and Rehabilitation setting.

As a member of our interdisciplinary therapy team, you'll help residents regain strength, mobility, and independence while delivering exceptional, resident-centered care.
ResponsibilitiesProvide skilled physical therapy treatments under the direction and supervision of a licensed Physical Therapist.
Implement individualized treatment plans for short-term rehabilitation and long-term care residents.
Assist residents in improving mobility, balance, strength, endurance, gait, and overall functional independence.
Monitor resident progress and communicate changes to the supervising Physical Therapist.
Educate residents and caregivers on therapeutic exercises, mobility techniques, and fall prevention strategies.
Complete accurate and timely documentation using Net Health.
Collaborate with nursing staff, physicians, and the interdisciplinary care team to achieve the best possible resident outcomes.
Maintain compliance with all federal, state, and facility regulations.
Current Pennsylvania Physical Therapist Assistant license or Indirect License eligible.
Graduate of an accredited Physical Therapist Assistant program.
Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but new graduates are encouraged to apply.
Experience with Net Health is a plus.
Strong communication, organization, and teamwork skills.
