Wyomissing Health & Rehabilitation Center is seeking a compassionate and dedicated Part-Time Speech-Language Pathologist (SLP-CCC) to join our in-house rehabilitation team in Reading, Pennsylvania. This is an outstanding opportunity for a Speech-Language Pathologist who is passionate about providing high-quality, patient-centered care in a Skilled Nursing and Rehabilitation setting.
As part of our interdisciplinary rehabilitation team, you'll have the opportunity to make a meaningful impact by helping residents improve communication, cognition, and swallowing function while enhancing their overall quality of life.
ResponsibilitiesEvaluate residents with speech, language, cognitive-communication, voice, and swallowing disorders.
Develop and implement individualized treatment plans to maximize functional outcomes.
Provide skilled speech therapy services to short-term rehabilitation and long-term care residents.
Perform dysphagia assessments and implement appropriate swallowing interventions.
Collaborate with physicians, nursing staff, dietitians, and the interdisciplinary care team to ensure optimal resident outcomes.
Educate residents, families, and caregivers regarding treatment plans and communication strategies.
Complete accurate and timely documentation using Net Health and Point Click Care (PCC).
Maintain compliance with all federal, state, and facility regulations.
Current Pennsylvania Speech-Language Pathologist license.
Certificate of Clinical Competence (CCC-SLP) required.
Master's degree in Speech-Language Pathology from an accredited program.
Skilled Nursing Facility (SNF) or Long-Term Care experience preferred, but not required.
Experience with Net Health and Point Click Care (PCC) is a plus.
Strong clinical assessment, communication, and organizational skills.
