Working to meet the health and nutritional needs of women, children and families with information, education, and quality care, MFHS is currently seeking a full time Maternal Health Advocate for Bradford and Tioga counties.

Job Summary

The Maternal Health Advocate is responsible for connecting MFHS clients with needed programs and services offered by MFHS and/or community partners and is key to facilitating healthy birth outcomes by providing education, support, and guidance to pregnant clients, and removing barriers to obtaining prenatal, perinatal, and postpartum services. This position is instrumental in promoting and achieving service integration across MFHS programs.

The Maternal Health Advocate will build effective working relationships with MFHS clients and community partners and educate and assist clients with navigating through the available perinatal, obstetric, and postpartum services available.

ESSENTIAL DUTIES AND RESPONSIBILITIES
  • Care Navigation - Connect clients to care and aid clients in navigating and accessing health and social services needed for improved health outcomes, with a focus on maternal health. Assist with fielding calls to the MFHS care navigation line and providing information on MFHS programs and community providers and resources, facilitate internal and external referrals for services, evaluate client eligibility and follow up on referrals.

    Assist clients with completing insurance applications, appointment scheduling, arranging for transportation assistance, and enrollment in public assistance programs.
  • Preparing Clients for Appointments – Provides appointment reminders, information on eligibility and required documentation needed for appointments, and payment reminders; completes insurance pre-certifications and makes missed appointment and appointment rescheduling calls.
  • Promote Service Integration - Collaborates with MFHS staff to connect clients across all MFHS programs and coordinates care through case management support and referral follow-up. Demonstrates an in-depth knowledge of programs and services offered by MFHS and MFHS subrecipients. Maintains effective communication and builds professional relationships with clients and co-workers to aid communication and eliminate barriers to care. Documents referrals and navigation services provided through tracking forms, electronic referral forms, and electronic medical records.
  • Engage with Community Partners - Engages with existing community partners and develops new partnerships across diverse fields (including health care, mental health/behavioral health, education, career services, criminal justice, children and youth, housing, food assistance, domestic violence prevention, and other social services), building strong professional alliances, accumulating extensive knowledge of the services available through them, and maintaining a database of the resources and services offered.
  • Assist with Outreach - Participates in professional conferences, community and MFHS sponsored events, and other meetings to promote awareness and referrals to MFHS programs and build relationships with community providers and resources.
