Rise Me is seeking a Population Health Coordinator to support hybrid community health, care coordination, and program implementation across the FPCSN network in Philadelphia. The role combines frontline health worker activities with program administration under the Population Health Manager’s guidance.
Initial focus includes Title V CYSHCN and FOCUS Grant initiatives, with opportunities to support additional grants, outreach projects, and data-driven quality improvements across sites.
