Serenity Hospice PA is seeking a compassionate and dedicated Hospice RN Case Manager (RNCM) to join our growing team serving patients throughout Dauphin County, PA.

This position offers the opportunity to make a meaningful impact by providing compassionate end-of-life care and support to patients and their families in the comfort of their homes.

• Manage and coordinate hospice patient care plans
• Perform comprehensive nursing assessments
• Provide symptom management and patient education
• Collaborate with physicians, caregivers, and interdisciplinary team members
• Maintain accurate and timely documentation
• Support patients and families emotionally throughout the hospice journey

• Active Pennsylvania RN license
• Hospice or home health experience preferred
• Strong clinical and organizational skills
• Excellent communication and compassionate patient care abilities
• Valid driver’s license and reliable transportation
