Job Title: Licensed Practical Nurse – LPN

Location: Lebanon, TN

Job Type: Full‑Time

The LPN is responsible for providing quality patient care and performing skilled care in the patient’s home. They provide services within the scope of practice as defined by state laws, follow the plan of care, and coordinate with other members of the health care team. They instruct families, maintain a healthy environment, observe, record, and report patient responses, collaborate closely with physicians and RNs, document care accurately and promptly, and may not alter the plan of care.

• One year of clinical LPN experience.
• Current LPN license in the state where services are rendered.
• Current CPR certification, driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.
• Ability to reliably commute to multiple patient residences daily within the assigned geographic area.
