- Review charts in the office to ensure documentation is complete, accurate, and compliant with regulations and payer standards.
- Collaborate with coordinators, administrators, and the clinical team to resolve issues, support staffing, and ensure continuity of care.
- Communicate with physicians, case managers, and other providers regarding patient status, care needs, and plan-of-care changes.
- Provide education and support to caregivers and families on the child's care needs, equipment, and safety.
- Offer clinical guidance to direct care workers/home care aides working with pediatric clients.
- Active Registered Nurse (RN) license in good standing in the state of Pennsylvania.

- Valid driver's license, reliable transportation, and willingness to travel to patients' homes within the service area.
- Strong documentation skills and comfort using EMR/electronic documentation systems.
- Ability to work independently in the field while also functioning as part of a team in the office.
- Excellent communication, organization, and time-management skills.
- Competitive pay, based on experience
- Hybrid schedule (field and office work)
- Medical, dental, and vision insurance
- 401(k)
- Flexible Spending Account (FSA)
- Referral bonuses
- Supportive leadership and a collaborative, family-focused work environment
