We are hiring Per Diem dedicated Hospice Nurse Practitioners to deliver exceptional patient care in and around the Wayne Area. As a Hospice Nurse Practitioner with us, you'll play a vital role in providing comprehensive hospice care. Your responsibilities include evaluating and assessing health histories, promoting health and well‑being, diagnosing medical conditions, and managing health problems by developing personalized care plans.

You'll collaborate closely with patients, families, and healthcare practitioners to ensure a holistic and supportive approach.

• Deliver compassionate medical care, providing palliative and terminal illness management, ensuring compliance with the state Nurse Practice Act under physician supervision.
• Collaborate for holistic patient care: consult with attending physicians, the hospice medical director, and other healthcare practitioners to assess eligibility and ongoing care needs.
• Coordinate and oversee patient care: review clinical information and coordinate with the interdisciplinary team and other practitioners, ensuring a well‑coordinated patient‑centric approach.
• Educate and counsel: engage with patients and families, addressing preventative care, medical, psychological, and spiritual concerns, and identifying care goals with the team.
• Recommend certification and recertification: provide input to the hospice medical director regarding patient eligibility for initial certification and recertification.
• Stay informed and compliant: maintain up‑to‑date knowledge of Medicare/Medicaid, state, and federal hospice regulations, ensuring strict compliance.

• Master’s degree with a minimum of one year of hospice or palliative care experience as an RN or NP preferred.
• Current unencumbered license in the state of practice as a Registered Nurse and Nurse Practitioner.
• Certification in a specialist area preferred (e.g., Hospice and Palliative Nursing (CHPN), Pediatrics, or Geriatrics).
• Excellent verbal and written communication abilities to articulate complex problems.
• Computer proficiency in typing and various programs, including EMR experience.
• CPR certification if required by state.
• Current driver’s license, vehicle insurance, and dependable transportation.

Per Diem Nurse Practitioners are paid $150 per visit.
