Position: LPN Ambulatory (PT-Day, 20hrs/week) - OB/GYN-General - Abington, PA

Summary

With a commitment to patient‑ and family‑centered care, the LPN uses the nursing process to value each individual’s uniqueness and address the physical, psychological, emotional, and social needs of the diverse patients served by Jefferson Health. The LPN functions as a member of the health‑care team, exercising sound nursing judgment based on preparation, knowledge, experience, and competency. The LPN participates in the planning, implementation, and evaluation of nursing care using focused assessment, and under the direction and supervision of a licensed clinical team member, carries out selected nursing acts that effectively address the individual needs and priorities of the patient and are consistent with their cultural beliefs and values.

Responsibilities
  • Interact with co‑workers, patients, their family/designated care partners, and other staff consistent with Jefferson values.
  • Integrate evidence‑based practice, clinical expertise, patient preferences, and cultural beliefs into the delivery of optimal healthcare.
  • Partner with patients and their family/designated care partners to identify learning needs and priorities, providing education that is useful, understandable, and affirming.
  • Recognize patient families/designated care partners in providing safe, compassionate, and coordinated care based on respect for patients’ personal preferences, priorities, needs, and cultural beliefs.
  • Function effectively within nursing and inter‑professional teams, fostering open communication, mutual respect, and shared decision‑making to achieve quality patient care.
  • Create an environment of belonging and inclusion, demonstrating skills in problem solving and conflict resolution.
  • Actively participate in practice setting‑based committees, including safety and quality improvement.
  • Demonstrate willingness to give and receive feedback, integrating appropriate changes into practice.
  • Use information technology to communicate, manage knowledge, mitigate error, and support ethical decision‑making.
  • Demonstrate a commitment to learning, seeking opportunities for continued professional growth and development, and act in a primary preceptor role for appropriate staff members.
  • Achieve and maintain compliance with all institutional policies and guidelines related to evidence‑based best practices for patient care.
  • Minimize risk of harm to patients and others by following established nursing practices, protocols, and individual performance standards.
  • Use data to monitor outcomes of care processes and apply improvement methods to design and test changes that improve quality and safety of patient care.
  • Communicate with a registered nurse, physician, and the patient’s health‑care team members to seek guidance with patient care.
  • Collect data as allowed by the licensed practical nurse scope of practice defined by state law.
  • Work closely with the registered nurse to ensure the implementation of safe, quality care for emergency department patients.
Qualifications
  • Specialized Diploma Graduate of an accredited School of Practical Nursing and passed the Pennsylvania Board Association for Licensed Practical Nurse.
  • Knowledge of patient‐ and family‑centered practice, including conveying respect and collaborating with each patient, family, and/or designated care partner in care planning.
  • Cultural awareness: knowledge of people with diverse backgrounds and the ability to convey respect and connect effectively.
  • Effective communication: understanding of communication concepts, tools, and techniques; ability to transmit, receive, and interpret ideas accurately; using partnership language with patients and families and delegating respectfully to team members.
  • Knowledge of patient evaluation and care, including disorders and relevant clinical conditions.
