Position Overview
Join a team of passionate and hard‑working individuals at Allegheny County Department of Human Services (DHS), Area Agency on Aging (AAA). This Aging Care Manager 3 position in our Community Based Services Division, Assessment Unit strengthens the safety net for elderly residents who need expert support. We seek self‑starters who can adapt and work with minimal supervision.
Description of WorkThis position performs advanced complex case management and referral functions, offers services to help eliminate risk, and works with consumers, families, and community partners to ensure consumer well‑being.
Duties include:
- Review Referrals:
Evaluate medical, social, psychological, and other factors for consumers referred for assessment, including advanced assessments on critical cases. - Complete Assessments:
Perform on‑site, face‑to‑face assessments of homebound, frail, elderly consumers in hospitals, care facilities, and their own homes. - Determine Risk:
Identify risk, including in‑depth abuse factors or danger to life and health, and coordinate next steps with supervisors and protective services. - Audit Cases:
Conduct detailed chart and desk reviews, prepare for, attend, and testify at appeal hearings. - Coordinate Communication:
Act as liaison for DHS/AAA with social service agencies, medical facilities, and community organizations; answer calls, emails, and questions from consumers, families, and facility staff. - Support Team Development:
Provide training, assistance, and mentoring to other caseworkers; interact with outside agencies and providers for outreach, training, and education.
- Full‑time employment.
- Work hours:
8:30 AM–4:30 PM, Monday‑Friday, with a 60‑minute lunch. - Telework: up to two days per week after six months, subject to having secure high‑speed internet and an approved Pennsylvania location. Otherwise, report to headquarters in Pittsburgh.
- Salary: starting salary may be non‑negotiable in some cases.
- One year as an Aging Care Manager 2 or Aging Program Assessor; or
- One and one‑half years of professional human services, law enforcement, or nursing experience; and a bachelor’s degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training that includes six months of professional human services, law enforcement, or nursing experience.
- Meet the PA residency requirement.
- Hiring preference may be given to candidates who live within Allegheny County.
- Must be able to perform essential job functions.
- This position falls under the provisions of the Older Adult Protective Services Act.
- A conditional offer of employment will require submission and approval of satisfactory criminal history reports, including PA State Police and FBI clearance.
