Position: Social Worker Children & Young People Disability Team
Location: Old Warren

Our client, Reading Borough Council is looking for a Qualified Social Worker to join their Children & Young People Disability Team.

The work style is hybrid with SW in the office twice a week.

Responsibilities
  • Conduct thorough assessments of children and young people, ensuring their needs are identified and met.
  • Arrange appropriate accommodation and provide interventions that safeguard and promote their welfare.
  • Adhere to company policies, procedures, and best practice guidelines to deliver high-quality outcomes.
  • Engage directly with children, families, carers, and communities, supporting them to make informed decisions and achieve positive change.
  • Build strong, trusting relationships and work collaboratively with colleagues and external agencies to gather information, plan care, and deliver effective interventions.
  • Maintain accurate case notes, prepare detailed reports, and present evidence in court when required.
  • Work closely with OT and other health colleagues to ensure the needs of the children are met, often leading in planning and reviewing cases for children in need, looked after and children subject to child protection.
  • Act as the lead professional in multi‑agency meetings, monitor progress and ensure objectives are met.
  • Recognise and assess risks promptly and elevate concerns to supervisors to implement mitigating actions.
  • Identify and address mental health concerns as part of holistic care planning, with experience in undertaking Mental Health Assessments required.
  • Stay informed about current legislation, policy changes, and evidence‑based practice; attend relevant training and development opportunities to maintain professional standards.
  • Promote equality, diversity, and health and safety compliance while demonstrating flexibility to adapt to changing demands within the organisation.
