Job Summary
Under the supervision of the Program Director, the Case Manager’s primary role is to work alongside families in Family Promise’s Shelter, Prevention, and Diversion programs. To help clients acquire gainful employment, maintain savings, and obtain permanent housing, as well as other tools of self-sufficiency. The Case Manager will work alongside the Family Promise and Lunch Break teams and partner agencies in the Monmouth County area to assist and obtain resources for families in our programs.
In addition to working with families currently in our program, the Case Manager will serve as a family advocate for alumni of the program.
Job Responsibilities
- With the FP team, maintain an active and ongoing waitlist of potential candidates for programs.
- Conduct prospective family screenings, intake, interviews, and selection in collaboration with the Program Director and Family Promise team.
- Ensure family composition data is entered into The Homeless Family Information System (HMIS), and appropriate databases, upon intake and discharge from the shelter program.
- Maintain statistics and prepare reports regarding the clients that are impacted by our programs
- Direct applicants and families to other services outside of Family Promise when appropriate.
- Responsible for dwelling visitations to ensure compliance with the rules of the shelter program.
- Provide intensive Case Management. Areas of potential assessment may include, but are not limited to:
- Life coaching, counseling, and mentoring.
- Monthly progress evaluations to determine eligibility in our programs
- Budgeting and financial management
- Budget projections for housing sustainability
- Family relations and support.
- Connect and mediate families with appropriate supportive services and housing resources.
- Refer adult participants to our Life Skills Program and coordinate management of services with the Case Manager and Life Coach.
- Maintain communication with the Program Director and housing resources in the area to obtain shelter placements.
- Coordinate services and resources for teens and children in our shelter programs.
- Create and maintain a list of effective housing resources and develop positive relationships so that these partnerships flourish and we become a preferred provider of their services.
- Help develop a blueprint for how our services will be delivered for our interim housing model and for the eventual 24 – unit affordable apartment building we plan on building.
- Help develop a better system and process to help our clients identify and apply for affordable housing as these opportunities become available; ensuring our clients are ready with all the documentation and records they need to apply.
- Facilitate childcare resources and school enrollment alongside McKinney-Vento Liaisons.
- Ensure case notes, family files, and statistical records are up-to-date and complete, and placed in the appropriate locations.
- Responsible for coordinating family transportation and meal delivery based on location.
- Connect with graduate families for updates and to keep them connected with other Family Promise graduates, as well as provide life counseling and coaching to be resilient and maintain stability.
- After-hours and weekend work is required on occasion.
- On-call duties for families in the program, such as communication outside of regular work hours.

- Bachelor’s degree (or greater) from an accredited college/university preferably in social work or a related field
- 3+ years of relevant experience in the field of homeless/human services.
- Experience or knowledge of the challenges associated with homelessness, including mental illness, substance abuse, violence, and isolation.
- Experience or knowledge of the principles and techniques of “beginning where someone is at,” engaging participants, establishing rapport, and building relationships.
