PRN Registered Nurse

The Registered Nurse (Home Health) will provide direct nursing care in patients’ homes, coordinate care plans, and ensure compliance with state and federal regulations.

• Clinical Competence
  • Initiate, develop, implement, and revise plans of care in collaboration with physicians and other health professionals.
  • Supervise home‑health aides and licensed practical/vocational nurses, providing instruction and assigning tasks per regulations.
  • Complete required supervisory visits.
• Documentation and Care Delivery
  • Provide high‑quality clinical services within scope of practice and infection control standards.
  • Complete comprehensive OASIS assessments and medication reconciliation accurately and timely.
  • Document patient visits per policy and payer requirements, ensuring timely synchronization with LHC policy.
• Quality
  • Conduct nursing evaluation visits, ensuring eligibility and medical necessity, and develop plans of care within state guidelines.
  • Communicate information promptly to agency staff, physicians, patients, families, and other clinicians.
  • Maintain quality care and service excellence through effective communication.
• Teamwork
  • Takes direction from Clinical Director and Executive Director, completing assigned tasks timely.
  • Assists in orientation of new personnel and serves as preceptor to other staff and students.
  • Participates in survey readiness activities, performance improvement plans, and patient safety initiatives.
  • Participates in on‑call and weekend rotations as needed.
  • Adheres to the agency’s utilization management model.
• Ability to function in any home situation, regardless of client characteristics.

• Current and unrestricted RN licensure in the state of practice.
• Valid driver’s license, vehicle insurance, and access to a dependable vehicle or public transportation.

• CPR certification or ability to complete within 90 days of hire.
• Ability to work independently.
• Strong communication, writing, and organizational skills.

• $67,905 – $101,857 annual total cash target pay.
• $32.65 – $48.97 per visit point.
• $39.18 – $58.76 hourly rate.
