Job Description:

Seeking a Physical Therapist to provide home health therapy services. Responsibilities include performing SOC visits, particularly for orthopedic patients without nursing orders, and managing an average daily census of 78 patients.

Responsibilities:

• Perform assessment visits and provide care for orthopedic and geriatric patients with co-morbidities such as CHF and COPD.
• Complete five patient visits per day and document care using the Netsmart EMR system.
• Work with intake RN and office manager to coordinate patient care.
• Provide occasional Saturday coverage for orthopedic patients when needed.

Requirements:

• Current certification in BLS.
• Active licensure to practice as a Physical Therapist.
• Experience with Netsmart EMR is preferred.
• Willingness to travel approximately 40 miles per day within Tehama County.
