About the PositionTravel Nurse Source is working with United Health Care Staffing, Inc. to find a qualified Home Health RN in Red Bluff, California, 96080!
Case Management: Conduct home visits to manage an assigned adult/geriatric patient caseload (typically 5 visits per day or ~5.25 productivity points).
Clinical Assessments: Perform admissions (Start of Care), Recertifications, Resumptions of Care, and Routine visits.
Skilled Care: Administer wound care/wound vacs, manage IV therapy (PICC lines), and monitor chronic conditions (CHF, COPD) to prevent rehospitalization.
Documentation: Complete accurate electronic charting using specialized home health software within mandatory compliance windows.

RequirementsLicense: Active, unrestricted California Registered Nurse (RN) license.
Experience: Minimum 1 to 2 years of recent, dedicated home health experience.
Software Skill: High proficiency with electronic medical records (Oasis and Home Care Home Base / HCHB strongly preferred).
Logistics: Valid U.S. driver’s license, reliable personal vehicle, and active auto insurance.
Life Support: Active AHA BLS certification.
