Job Title

Registered Nurse (RN) | Case Manager | $5k Sign‑On

Annual Salary: $85,000. Sign‑On Bonus: $5,000. Productivity Bonuses: $100 per additional regular visit, $150 per SOC.

Schedule:

Monday – Friday, 8:00 a.m. – 5:00 p.m.

Location:

Redding, CA, and surrounding areas.

Provide in‑home direct patient care according to the provider’s prescribed plan of care (and nursing scope of practice) while closely monitoring progress/recovery.

• Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.).
• Administer medication, insulin, and IV/fluids, documenting thoroughly.
• Inspect and care for wounds, changing dressings and assisting with personal hygiene.
• Assess patient for further injuries, infections, bedsores, etc. and deliver appropriate treatments.
• Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
• Participate in ongoing interdisciplinary assessment of the patient.
• Participate in Quality Assessment and Performance Improvement (QAPI) program.
• Supervise paraprofessional and licensed professional team members.

• Graduate of an accredited school of nursing and has a current license as a Registered Nurse in CA.
• Two (2) years of current nursing experience preferred.
• One (1) year management experience preferred.
• Must have Home Health or Hospice experience.
• Community Health and a wide range of Medical Surgical experience preferred, not required.
• Ability to pass a Criminal Background Check, Physical, TB & Drug Screen.
• Must have a current Driver’s License and reliable transportation.
