Goodhue County Health & Human Services is seeking qualified applicants for a Care Coordinator.

Provide case management, care coordination and comprehensive assessments to Goodhue County residents.

• Complete MN-Choices or assessments in home and community settings.
• Assess and evaluate client’s health and living situations to determine level of care and services needed.
• Develop person-centered care plans to address service needs in the community.
• Collaborate with community providers to meet client needs.
• Provide input in program planning, the development of agency goals and the evaluation of the agency’s programs and policies.
• Work as part of the Home and Community Based Services and Health Plan team.

• Bachelor’s degree in nursing with certification as a public health nurse and current licensure as a registered nurse in Minnesota OR a current licensure as a registered nurse.
• OR Bachelor’s degree in social work, psychology, sociology or other closely related human services field.
• Must possess a valid driver’s license. Ability to complete and pass MNCHOICES certification upon hire.

County employees receive competitive pay and a generous benefit offering. The Care Coordinator is compensated at salary grade 112 and starting annual compensation is $73,860.80.
