Position Overview:
The SLP will assess, diagnose, and treat speech, language, and swallowing disorders while developing personalized treatment plans that support each patient’s rehabilitation goals. You’ll work closely with patients and their families to promote independence and enhance communication skills in a home environment.
Key Responsibilities:
• Provide effective patient care using approved therapy techniques and procedures
• Evaluate patients’ functional levels, treatment needs, and progress toward goals
• Develop and implement individualized treatment plans in accordance with physician orders
• Supervise Speech Therapy Assistants (STAs) and coordinate patient care
• Document treatments, progress notes, and care plan updates accurately and on time
• Assist patients and families in developing and reviewing home programs
• Recommend adaptive equipment and evaluate home settings when needed
• Maintain patient confidentiality and compliance with HIPAA regulations
• Collaborate with other healthcare providers to ensure coordinated, high-quality care
• Complete discharge summaries and obtain necessary orders at case completion
Qualifications:
• Licensed Speech Language Pathologist (SLP) in the CA
• Minimum of 1 year of SLP experience preferred
• Current physical (within the last 6 months)
• Current TB test (within the last 12 months) and CPR certification
• Valid driver’s license and reliable transportation
• Ability to attend a 1-3-day orientation at our Yuba City office
