Registered Nurse (RN) | Case Manager | $5k Sign‑On
Salary: $85K & $5k Sign‑On Bonus
Productivity Bonuses: $100 per additional regular visit, $150 per SOC.

Monday – Friday, 8a – 5p.

Must have Home Health or Hospice Experience
As a Registered Nurse (RN) / Case Manager, you will be called to care when you are needed most. You will support a full range of patient services to bring comfort and dignity to our clients.

- 1:1 nurse‑to‑patient ratios.
- Ability to create your own schedule.
- Provide in‑home direct patient care per the provider’s prescribed plan of care and nursing scope of practice, monitoring progress and recovery.
- Assess and document patient conditions and report changes as needed (vitals, blood/stool/urine, glucose, etc.).
- Administer medication, insulin, and IV/fluids, documenting thoroughly.
- Inspect and care for wounds, change dressings, and assist with personal hygiene.
- Assess patients for injuries, infections, bedsores and deliver appropriate treatments.
- Collaborate with providers, aides, and allied health professionals to ensure exceptional patient outcomes.
- Participate in ongoing interdisciplinary assessment of patients.
- Participate in Quality Assessment and Performance Improvement (QAPI) program.
- Supervise paraprofessional and licensed professional team members.
- Graduate of an accredited school of nursing with a current license as a Registered Nurse in CA.
- Two (2) years of current nursing experience preferred.
- One (1) year management experience preferred.
- Must have Home Health or Hospice experience.
- Community health and a wide range of medical‑surgical experience preferred, not required.
- Ability to pass a criminal background check, physical, TB & drug screen.
- Current driver’s license and reliable transportation.
