As the Home and Community Based Services Licensed Practical Nurse, you will provide direct nursing care as prescribed by the physician to acutely and/or chronically ill patients in their homes, record clinical and progress notes, and coordinate other patient care activities/services to deliver the highest quality of patient care in your assigned geographic service area. You will work under the direct supervision of the Registered Nurse and/or designated supervisor.

Responsibilities
  • Provide high quality clinical services within the scope of practice, as defined by state laws governing the practice of nursing, in accordance with the plan of care and in coordination with other members of the health care team, following all applicable infection control procedures.
  • Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable).
  • Actively participate in the agency's care coordination process including timely reporting of patient needs and changes in condition, attending patient case conferences, and communicating effectively with the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers (as applicable).
  • Operate effectively in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Required Qualifications
  • Current and unrestricted Licensed Practical Nurse licensure in the state of California.
  • Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
State Specific Requirements
  • California: 1+ years of professional nursing experience as an LPN.
Preferred Qualifications
  • Current CPR certification or ability to complete within 90 days of hire.
  • Ability to work flexible hours as required to meet identified client needs.
  • Ability to manage multiple tasks simultaneously.
  • Able to work independently.
  • Good communication, writing, and organizational skills.
Pay Details

The hourly pay for this role will range from $52,668 to $79,002 per hour based on full-time employment. Salary is based on several factors including but not limited to local labor markets, education, work experience, certifications, and other considerations.
