Job Summary:

As an Evening Shift LPN, you will play a vital role in creating a warm and inclusive environment for our residents. We are seeking a compassionate, skilled, and dedicated LPN to join our dynamic team.
Responsibilities:
- Provide direct care to residents, including administering medications, performing vital signs, and assisting with daily living activities
- Develop and maintain personalized care plans, working closely with the care team to ensure resident needs are met
- Assist with mealtime, including serving, feeding, and ensuring hydration needs are met
- Participate in care conferences and contribute to resident assessments, care plans, and service plans
- Maintain accurate and timely records of resident care and services
- Collaborate with other healthcare professionals, including physicians, CNAs, and staff members to ensure high-quality care
- Engage with residents, families, and staff members to promote a positive and inclusive environment
- Perform other duties as assigned by the shift leader or director of nursing

Preferred Qualifications:
- Current LPN/LVN license in the state of California
- One year of experience in a long-term care setting
- Basic Life Support (BLS) certification
- Ability to lift, push, and pull up to 50 pounds
- Strong communication and interpersonal skills
