As the direct supervisor for the Homeless Outreach team, this role provides steady, supportive leadership while also coordinating cross-department efforts within the City’s innovative THRIVE:
Community Health Program. The Community Health Program includes the Homeless Outreach, Mobile Integrated Health, and Mental Health co-response team, whose goal is to provide safety, stability, opportunity, and hope for anyone in need or crisis. With a focus on guiding staff, fostering collaboration, and ensuring alignment with broader departmental goals, the Community Health Manager helps create responsive, effective strategies to meet community needs and helps ensure the Community Health teams are providing cohesive, coordinated, and effective response to community needs and working together seamlessly.

Supervise and guide the daily activities of Homeless Outreach Team to ensure effective outreach, intervention and follow-up services are provided to individuals who are unhoused or at risk of losing their housing. Leads aspects of the City’s THRIVE program which consists of homeless outreach, community court, human services, mobile integrated health and mental health co-response. Coordinate efforts among the planning, police, and fire departments, as well as community and regional partners, to support behavioral health, homelessness response, chronic illness, and other crisis-related services.

This position plays a critical role in ensuring clear communication and information-sharing across all THRIVE program partners. The Community Health Manager is responsible for maintaining up-to-date awareness of client needs, staff workload, emerging issues, and follow-up tasks, and for making sure this information is shared promptly with internal departments and external service partners. By providing consistent updates, tracking progress, and facilitating collaboration, this role ensures that outreach efforts are aligned, gaps are identified early, and teams can respond cohesively to support individuals and families in crisis.

  • Supervise and guide the daily activities of Homeless Outreach staff to ensure effective response.
  • Provide guidance and support to Community Health teams handling mental, behavioral, social, or emotional crises. Lead case-conferencing to collaborate on the development of recommended care plans, resource placement and follow-up strategies, and handle complex cases.
  • Maintain resources for mental health, behavioral health, homelessness, and other needs, actively seeking out new resources that will support the Community Health teams’ goals.
  • Guide Community Health team members on appropriate referrals and continuously evaluate resource effectiveness by tracking client outcomes.
  • Recommend and implement updates to policies and protocols to strengthen a coordinated response.
  • Ensure staff compliance through coaching, guidance, and clear communication, and address performance issues by outlining necessary corrective actions.
  • Maintain communication with stakeholders, including service providers, community organizations, and City departments.
  • Oversee documentation of client interactions, assessments, and service plans to ensure compliance with standards.
  • Develop reports and provide regular updates on team activities, resources, and outcomes to City leadership.
