The Homeless and Housing Stability Program Coordinator provides housing navigation for and outreach to individuals and families who are at risk of or experiencing homelessness in the City of Redmond.

This position serves as part of the City's Outreach Response Team within the Human Services Division working alongside the Homeless Outreach Administrator to provide direct outreach and comprehensive case management support.

The Program Coordinator is a human services professional who can collaborate with local non-profits to connect clients to resources and support the overall goals of the City's THRIVE:
Community Health Team by providing safety, stability, opportunity, and hope for anyone in need or crisis.

ROLE & RESPONSIBILITIES
  • Primary responsibility will be to provide support to the Homeless Outreach Program which includes connecting individuals experiencing or at risk of homelessness with local provider resources and coordinating other administrative and service needs. This will include direct outreach and in-person support to clients.
  • Assist individuals in accessing appropriate service providers and coordinating service delivery, including but not limited to connection to behavioral health services, housing, treatment services, employment services, basic needs services, etc. This includes providing direct transportation to shelters, appointments, and other services.
  • Provide back-up support for the City of Redmond's THRIVE Program which also includes a team of individuals from departments overseeing the City's Mobile Integrated Health Program and Mental Health Professional Co-Response.
  • Conduct assessments, document client interactions, and assist with social services, benefits, and housing applications.
  • Support individuals at risk of losing their housing with identifying short and long-term solutions that will prevent eviction and/or homelessness.
  • Participate in cross-departmental case consultations and coordinate with cross-department staff, including Parks, Police, and Fire, to address homelessness and housing related issues.
  • Coordinate and collaborate with external community partners and agencies to make referrals and connections. Works proactively with local and regional outreach teams, housing providers, and human services organizations to understand community resources.
  • Stay apprised of services, programs, and eligibility criteria for accessing resources.
  • Identifies gaps and provides recommendations to shape the city's response to homelessness and housing instability.
  • Conduct follow-up visits as necessary to ensure the continuity of care and support the transition to stability and independence.
  • Maintain accurate records, data, and materials related to client interactions, ensuring comprehensive documentation and case management.

  • Skills in conflict resolution, de-escalation techniques, motivational interviewing, and problem-solving.
