The Abigail, a 24-bed residential care home for seniors in Redwood City, is looking for a Direct Care Supervisor to oversee daily care operations for all residents including management of caregivers, assessing resident care needs, and communicating with staff, clinicians, and family members. This position plays a critical role in maintaining care quality, monitoring resident condition changes, and communicating clinical observations to appropriate management, clinicians, and family.
The Direct Care Supervisor combines hands-on caregiving expertise with care coordination and team leadership responsibilities. The role ensures residents’ health, safety, dignity, and wellbeing while supporting caregivers through coaching, training, and performance management.
This position is non-exempt (hourly + overtime; range is $35-$40/hr) and located on-site in Redwood City (Mon-Fri with a start time ~6-7am). Willingness to be on-call evenings, weekends, holidays.
Key Responsibilities Resident Care Management- Monitor resident health status, behavior, and overall wellbeing through regular observation and interaction.
- Evaluate changes in resident physical, cognitive, or emotional condition and communicate updates to clinicians, nurses, and family members.
- Coordinate care needs with physicians, nurses, therapists, and community partners.
- Ensure individualized care plans are followed and updated as resident needs change.
- Support care transitions, new resident onboarding, and service adjustments.
- Respond appropriately to resident incidents, emergencies, or care concerns.
- Maintain accurate documentation of resident care, observations, and communications.
- Directly supervise and support 12-15 caregivers assigned within the residential home.
- Provide day-to-day leadership, coaching, and guidance to staff.
- Monitor performance, professionalism, and adherence to care standards.
- Conduct shift oversight, rounding, and quality assurance checks.
- Support staffing coordination to ensure appropriate caregiver coverage.
- Address performance concerns.
- Assist with caregiver onboarding, training reinforcement, and skill development.
- Oversee medication assistance and administration practices in accordance with state regulations and organizational policy.
- Ensure proper medication documentation and reporting procedures are followed.
- Monitor residents for medication effectiveness, side effects, or adverse reactions.
- Escalate medication or health concerns to licensed clinicians as appropriate.
- Serve as a primary point of contact for families regarding day-to-day care updates.
- Provide timely and professional communication related to resident condition changes.
- Partner with clinicians and healthcare providers to support coordinated care delivery.
- Bachelor’s Degree; or 4+ years of relevant work experience
- 4+ years of direct client care / resident care management experience within home care or senior living communities
- Current or previous experience working directly as a licensed Caregiver or CNA
- Bilingual Spanish a plus
- Administrator Certificate a plus (or willingness to get licensed)
- Experience with medication assistance or overseeing medication management practices
- Strong observational skills with the ability to recognize changes in resident condition
- Strong communication skills with ability to effectively relay critical information to care providers, families and healthcare professionals
- Leadership or supervisory experience preferred
- Deep desire to serve the senior residents
- CPR/First Aid certification is a plus; CNA or Med Tech certification a plus
