Part‑time Registered Nurse Case Manager
Schedule:
Saturday/Sunday
Territory:
Redwood City‑Sunnyvale Area
- Conduct Start of Care (SOC) admissions and OASIS assessments.
- Conduct follow‑ups and evaluation visits as needed in the designated territory.
- Make home visits to patients within a 5‑15 mile radius.
- Perform assigned duties, including medication reconciliation, wound care, treatments, and procedures.
- Monitor patients’ conditions and report changes to the clinical team.
- Collaborate with physicians and agency staff to deliver the best care possible.
- Accurately document observations, interventions, and evaluations pertaining to patient care management and services provided.
- Use company‑issued tools such as an iPad and specialized software for documentation and communication.
- Maintain and update patient's home care plans and OASIS forms.
- Participate in interdisciplinary team meetings and care conferences.
- Communicate with patients and families to provide education and support.
- Assist with administrative tasks using concierge resources.
- Manage mileage and vehicle reimbursement as per company policy.
- Comply with all regulatory and agency standards.
- Current and unrestricted California Registered Nurse (RN) license.
- Current BLS/CPR certification.
- Valid California driver's license.
- Experience conducting SOC (OASIS) assessments.
- High level of clinical knowledge in home health care.
- Experience with high acuity patients is a plus.
- Experience working with an interdisciplinary team.
- Familiarity with case management fundamentals.
- Self‑directed, empathetic, and a critical thinker.
- Outstanding interpersonal skills.
- Leadership skills.
- Organized and detail oriented.
