Job Summary

Coordinates, facilitates, and performs an interdisciplinary plan of care through assessment and evaluation of patient condition and effectiveness of care under the direction of the attending physician by traveling to patient homes, Community‑Based Residential Facilities (CBRF), assisted living locations, hospitals, or other sites to meet patients and families/caregivers, assess patient care needs, and determine acuity in relation to policies, procedures, and regulations of home care and/or hospice services.

Arlington, Portage, Rio, Wyocena, Dane, Deforest, Doylestown, East/North Bristol, Lodi, Morrisonvilee, Windsor, Briggsvile, Cambria, Dalton, Endeavor, Fox Lake, Kingston, Lewiston, Marquette, Montello, Oxford, Packwaukee, Pardeville, Plainville, Westfield, Lake Mills, Lowell, Reesville, Waterloo, Beaver Dam, Columbus.

Schedule

Monday‑Friday, 8:00–4:30pm. Requires one weekday on‑call approximately every 2 weeks. No weekend work. Mileage reimbursement of $0.70 per mile paid every 2 weeks.

Primary Responsibilities
  • Assesses and coordinates patient discharge planning needs with members of the healthcare team.
  • Performs point‑of‑care testing according to policies and procedures.
  • Collaborates, consults, and communicates with physicians regarding the plan of care and changes to patient status.
  • Provides physicians with regular updates on progress and recommends revisions to the plan of care.
  • Documents patient status and communication in adherence to regulations.
  • Coordinates and communicates with multidisciplinary team members and outside agencies to achieve expected outcomes and to provide patient education through end of life or discharge.
  • Completes admission paperwork and verifies insurance information.
  • Completes a comprehensive medical assessment by entering data into the clinical computer program in a timely manner.
  • Provides skilled nursing care within the established plan of care, including vital sign measurement, wound care, IV care, ostomy care, blood draws, injections, and physical assessment of the overall patient’s medical condition.
  • Documents visit notes, treatments, and care. Follows visit patterns according to physician orders and ensures required visits are completed on time.
  • Responds to calls from patients, family members, and others, completing follow‑up to address needs, issues, or concerns.
  • Works in a constant state of alertness and safe manner and performs other duties as assigned.
Education and Experience
  • Graduate of an accredited school of nursing or holds an education equivalency for licensing.
  • No prior experience required.
Additional Requirements

Proof of motor vehicle liability insurance that meets minimum state requirements upon hire and upon request.

Physical Requirements

Constant use of speech, standing, walking, frequent lifting, and carrying of objects weighing 0-25lbs; frequent use of vision and depth perception at near and far ranges; frequent use of smell and hearing for communication and safety; frequent driving; frequent keyboard use/data entry; occasional lifting and moving of patients; occasional bending, stooping, kneeling, squatting, twisting, gripping; rare crawling and running. Must be able to perform all job functions while maintaining safety, quality, and communication standards.

Professional Licenses and Certifications
  • State of Illinois – Basic Life Support Health Care Provider (BLSHCP), Registered Professional Nurse (RN) issued by IDFPR, Motor Vehicle Report (MVR), Driver’s License – State Issued.
  • State of Missouri – BLSHCP, Registered Nurse (RN) issued by Compact State or Missouri RN, MVR, Driver’s License – State Issued.
  • State of Oklahoma – BLSHCP, RN issued by Compact State or Oklahoma Board of Nursing (OBN), MVR, Driver’s License – State Issued.
  • State of Wisconsin – BLSHCP, RN issued by Compact State or Wisconsin Department of Safety and Professional Services, MVR, Driver’s License – State Issued.
