Transitions At Home is currently seeking to hire a full‑time RN Case Manager in the Reedsburg area. Our growing home health agency is seeking a compassionate and experienced RN to coordinate and oversee patient care in the home setting, ensuring personalized, high‑quality outcomes for every client.

Transitions At Home Registered Nurse Case Managers provide skilled nursing care to include assessments, medications, treatments, and coordinate services. They counsel the patient and family/significant others in their place of residence in accordance with the patient’s physician orders and plan of care, under the direction and supervision of the RN Clinical Manager and Administrator.

• Full‑time: 40 hours per week, 8:00 am – 4:30 pm, Monday–Friday
• No weekends or holidays
• Limited on‑call rotation

• Active Wisconsin Registered Nurse license
• Two (2) years’ nursing experience, one year in home health (preferred)
• CPR certification
• A valid Wisconsin driver’s license and car insurance
