Position: LICENSED PRACTICAL NURSE

Cone Health is seeking a compassionate LPN for our Long-Term Care team within our Healthcare & Medical Services department. In this role, you will provide direct patient care, administer medications, monitor vital signs, and support care planning for residents in a collaborative, multidisciplinary environment. You’ll partner closely with RNs, physicians, and families to promote safety, comfort, and dignity. Current LPN license in North Carolina and BLS certification required; prior LTC experience preferred.

  • Provide direct, compassionate nursing care to residents in a long-term care setting under the direction of an RN and within the LPN scope of practice.
  • Administer medications and treatments safely and accurately, including oral, topical, and injectable medications per provider orders and Cone Health protocols.
  • Monitor, record, and report changes in residents' condition, including vital signs, pain levels, and response to treatments, to the supervising RN and provider team.
  • Perform skilled nursing procedures such as wound care, dressing changes, catheter care, and tracheostomy care as appropriate to the LTC environment and licensure.
  • Participate in development, implementation, and evaluation of individualized care plans in collaboration with RNs, providers, therapy staff, social work, and other members of the interdisciplinary team.
  • Support residents with activities of daily living (ADLs), ensuring safety, dignity, and respect while promoting maximum independence.
  • Document all care, assessments, interventions, and resident responses accurately and promptly in the electronic health record in accordance with regulatory and Cone Health standards.
  • Collaborate closely with CNAs and other support staff, providing direction, mentoring, and support to maintain high-quality, resident-centered care.
  • Educate residents and their families about medications, treatments, disease processes, and wellness strategies, promoting understanding and participation in care decisions.
  • Follow infection prevention and control protocols, including appropriate use of PPE and isolation precautions, to maintain a safe environment for residents and staff.
  • Respond effectively to urgent and emergent resident needs, using sound clinical judgment and Cone Health policies to elevate care appropriately.
  • Participate in quality improvement, safety initiatives, and regulatory readiness activities within the long-term care facility.

  • IV therapy certification or demonstrated competency, if required by facility practice and state scope of practice
