Job Summary:
Provide compassionate and skilled nursing care to geriatric patients in a long-term care or skilled nursing facility setting. Ensure patient safety, comfort, and compliance with regulatory standards during night shifts.
Responsibilities:
- Administer prescribed medications and treatments to geriatric patients.
- Monitor patient conditions, document observations, and report changes to healthcare team.
- Adhere to safety protocols including IVs, wound vacs, trachs, and central lines management.
- Collaborate with interdisciplinary teams to develop and implement individualized care plans.
- Float to different units as needed, maintaining flexibility in assignments.
- Complete modules and required training during orientation to ensure compliance.
Qualifications:
- Valid LVN/LPN license with active status.
- Minimum of 2 years of nursing experience in long-term care or skilled nursing facilities.
- BLS certification required.
- EPIC experience necessary for documentation and patient management.
- Ability to admit and manage complex procedures such as TPN, G-tubes, PROM machines, and IVs.
- Strong clinical skills with geriatric patient care and trauma management.
