Home Health Physical Therapist - per visit

Overview

BAYADA Home Health Care is seeking a Home Health Physical Therapist for a per diem opportunity in Reisterstown, Maryland. This position covers home health visits in West Baltimore County, including Reisterstown, Owings Mills, Randallstown, and Pikesville. Per diem requires current home health and Oasis experience. One year of prior clinical experience as a licensed Physical Therapist is required.

• Make home visits to clients in designated geographic territories.
• Perform diagnostic tests and measurements such as mobility/range of joints, transfer status, stability, ambulation patterns, strength and endurance, balance, and safety assessments.
• Implement integumentary protective interventions to prevent wound formation and promote wound repair using evidence-based dressings and topical agents.
• Develop and implement individualized care plans, including manual therapeutic exercises, gait training, balance, and other interventions.
• Continuously assess and revise the PT care plan and participate with nursing in the multidisciplinary care plan as appropriate.
• Educate clients and family members in rehabilitative care to promote health, safety, and independent living.
• Accurately document observations, interventions, and evaluations using an electronic device.

• Maryland Physical Therapist license (PT).
• Minimum of one year of recent work experience as a Physical Therapist - PT.
• Graduation from a program approved by the American Physical Therapy Association or equivalent.
• Ability to work independently and manage time effectively.
• Strong interpersonal, organizational, and problem-solving skills.
• Solid computer skills; prior experience with EMRs preferred.

Base Pay: $57.00 - $73.00 per hour/point, depending on qualifications. BAYADA complies with all state/local mandates and is an equal opportunity employer.
