The Medical Director works closely with the Chief Medical Officer (CMO), Chief Operating Officer (COO), Chief Executive Officer (CEO), clinical leadership, and interdisciplinary care teams to support high-quality clinical care and organizational priorities.

The Medical Director provides clinical leadership, guidance, and support across CHA's health centers and clinical programs. This position contributes to clinical strategy, quality improvement, provider development, care-model transformation, regulatory compliance, and the ongoing advancement of CHA's Patient-Centered Medical Home and team-based care model.

• Provide clinical leadership and guidance to CHA health center providers and interdisciplinary care teams.
• Promote consistent delivery of high-quality, evidence-based, patient-centered primary care.
• Support providers in addressing complex clinical cases, care coordination needs, and clinical practice questions.
• Promote effective collaboration among physicians, APPs, nurses, behavioral health professionals, care coordinators, pharmacists, dentists, and other members of the care team.
• Support appropriate clinical utilization, patient access, continuity of care, and population health initiatives.
• Maintain professional clinical competence consistent with licensure, certification, credentials, privileges, and scope of practice.
• Participate in clinical care, as applicable to the position and organizational needs.

The Medical Director may be a Physician (MD/DO) or Advanced Practice Provider (APP), consistent with organizational needs and applicable credentialing, licensure, and regulatory requirements.

When the Medical Director is a Physician (MD/DO), an essential function of the position may include responsibility for supervising, collaborating with, or otherwise providing required physician clinical oversight for up to three (3) Advanced Practice Providers at any one time. Advanced Practice Providers may include Physician Assistants, Nurse Practitioners, or other licensed clinicians for whom physician involvement is required or appropriate.

Responsibilities may include:

• Reviewing clinical care and documentation.
• Providing clinical consultation and guidance.
• Maintaining timely availability for clinical questions.
• Completing required chart reviews.
• Participating in quality assurance and quality improvement activities.
• Executing and maintaining supervision, collaboration, or other agreements and documentation required by law or organizational policy.

When the Medical Director is an APP, clinical leadership, collaboration, consultation, and oversight responsibilities will be performed within the individual's education, training, licensure, certification, credentials, privileges, and authorized scope of practice. An APP Medical Director will not be assigned physician-specific supervisory or collaboration responsibilities that require a physician under applicable law or regulation.

All supervisory, collaborative, and clinical oversight responsibilities will be performed in accordance with applicable federal and state laws, licensing-board requirements, payer requirements, medical-staff bylaws, organizational policies, the Medical Director's credentials and privileges, and the education, experience, and authorized scope of practice of each provider.
