JOB PURPOSE

The Ryan White North Non-Medical Case Manager is responsible for delivering high-quality, client-centered support services to individuals enrolled in Ryan White and HOPWA programs. This position plays a critical role in ensuring clients are effectively connected to medical care, insurance coverage, and essential supportive services while maintaining compliance with all applicable federal, state, and organizational guidelines.

The Case Manager conducts eligibility screenings, coordinates referrals, and provides services such as transportation, tax preparation, and food assistance. This role requires accurate documentation across systems. The ideal candidate is organized, adaptable, and able to manage multiple priorities while delivering compassionate, professional service.

KEY RESPONSIBILITIES AND

ACCOUNTABILITIES
  • Conduct eligibility screenings for Ryan White Part B and related programs.
  • Collect and review client information to determine eligibility for insurance coverage and establish payer of last resort, including Medicaid, Medicare, Marketplace, and employer-sponsored plans.
  • Educate clients on health insurance options, enrollment, and care coordination.
  • Coordinate referrals to medical care and supportive services, including transportation, food, and tax assistance.
  • Serve as a liaison between clients and AHN and community service providers to coordinate care and ensure access to services.
  • Follow up on services provided, including maintaining transportation logs and verifying completed trips.
  • Track appointments, monitor compliance, and follow up on services provided.
  • Maintain accurate and timely documentation in CAREWare, Salesforce, and other systems.
  • Process referrals and documentation within required timelines; maintain compliant client files.
  • Assist with insurance enrollment, eligibility determinations, and subsidy programs (APTC, CSR, HIP, MAP).
  • Monitor funding usage and process payments (e.g., transportation, premium assistance).
  • Respond to client and partner inquiries promptly and professionally.
  • Ensure compliance with HIPAA and program requirements (Ryan White, HOPWA).
  • Collaborate with staff and community partners; participate in trainings and meetings.
  • Adapt to changing program needs and support completion of assigned grant activities.
  • Perform other duties as assigned to support departmental and organizational goals.
EDUCATION AND EXPERIENCE
  • Bilingual (English/Spanish) required.
  • High school diploma or equivalent required;
    Bachelor's degree in healthcare, social services, or a related field preferred.
  • Minimum of 1–3 years of experience in healthcare, case management, insurance, or related field.
  • Community Health Worker (CHW) Certification (to be obtained within 1 year, agency-funded)
  • Strong computer skills, including proficiency in Microsoft Office (Word, Excel, Outlook) and experience with database systems.
  • Experience with data entry and the ability to consistently maintain accurate and detailed records.
  • Ability to manage workload, prioritize tasks, and meet deadlines in a fast-paced environment.
  • Experience with CAREWare, Salesforce, or similar systems.
  • Knowledge of Ryan White, Medicaid, Medicare, and Marketplace programs.
  • Experience with eligibility determinations and insurance enrollment.

PHYSICAL REQUIREMENTS
  • The noise level in the work environment is usually moderate, and the employee usually works in a climate-controlled office environment.
