Community Health Worker

• Provide care coordination and resource navigation to an assigned caseload of community member clients with unmet social needs
• Conduct telephonic and face-to-face outreach, follow-up, and coaching to assess needs, provide education, and assist with service enrollment
• Administer social determinants of health screenings, intake forms, and assessments in the Activate Care platform
• Help clients prioritize goals and create client-centered care plans
• Coordinate with community nonprofits and resources to address client needs
• Provide resources to improve clients' health literacy and self-sufficiency
• Coordinate healthcare transitions after hospital discharge or ER visits and collaborate with providers and families to ensure continuity of care and prevent readmissions
• Coordinate care across healthcare settings and facilitate specialty or behavioral program referrals
• Support Medicaid and health plan navigation, service authorizations, benefits and appeals education, and care coordination advocacy
• Provide person-centered coordination for high-risk members experiencing housing instability, complex medical conditions, or acute social barriers
• Assist with scheduling appointments, completing applications, and making reminder calls
• Maintain client privacy and confidentiality
• Participate in weekly team meetings, workshops, and training
• Commute to clients' homes and field locations as required
• Perform other duties as assigned

• Minimum of a high school diploma or equivalent
• Active Certified Community Health Worker I credential through the Nevada Certification Board, or willingness to obtain certification upon hire
• Valid driver's license in the state of Nevada
• Ability to use a personal vehicle to commute to and from clients' homes
• 2–3 years of relevant direct care coordination experience preferred
• Experience working with nonprofits, social service providers, faith-based groups, or government agencies addressing social determinants of health
• Strong independent working skills and communication
• Collaborative approach to supporting colleagues
• Cultural humility and ability to communicate effectively with diverse communities
• Additional language skills are a plus

Demonstrates expertise in care coordination and resource navigation for clients with unmet social needs, utilizing strong communication skills and cultural humility to effectively engage diverse communities. Proficient in administering assessments and facilitating healthcare transitions while maintaining client confidentiality.
