The National Organization of Nurse Practitioner Faculty is seeking a Hospice Nurse Practitioner to provide consultation in palliative care, symptom management, and supportive care for hospice patients as requested by the referring physician.
You will make face-to-face encounters within prescribed time limits, coordinate the plan of care with the IDG, document findings, and collaborate with the patient, family, attending physician, and other clinicians.
