Case Manager, Redfield Academy (Bilingual)

Bilingual Redfield Academy Case Manager

Pay Rate: $23.38-$28.04

Status:

Definition:

Redfield Case Managers provide wraparound case management services to youth attending Redfield Academy-an alternative education setting for Washoe County students at risk of dropping out of high school. These students have the opportunity to recover credits and work toward graduation with a traditional diploma, either at Redfield or through re-entry into a traditional school setting.

Case management services emphasize strengths, sustainable solutions, and community support. Case Managers assist youth and families through advocacy, crisis intervention, academic and graduation planning, and facilitation of support systems. Our wraparound model is family-driven, rooted in strengths, and supported by a collaborative team-based approach. Redfield Case Managers also work closely with school district personnel, teachers, and support staff to address barriers to student success.

Supervision:

This position has no direct supervisory responsibilities.

Duties:

Develop a thorough understanding of the agency's mission, strategic focus areas, and overall objectives, actively contributing to their success.

Read, understand, and consistently apply all agency and department policies and procedures, and adhere to ethical standards and applicable regulations.

Maintain a comprehensive knowledge of agency programs and community resources to provide accurate information and referrals during case management.

Provide wraparound case management services to youth and families, including intake assessments, individualized service planning, and ongoing progress monitoring.

Conduct intake assessments to evaluate clients' academic, emotional, and social needs, along with family dynamics.

Facilitate access to services and referrals, including mental health, housing, education, employment, and supportive resources.

Provide crisis intervention, de-escalation, and short-term stabilization services as needed.

Create and implement individualized service and graduation plans for students enrolled in Redfield Academy.

Monitor student progress, collaborate with teachers, and adjust support plans based on evolving needs.

Attend IEP meetings and advocate for student accommodations and supports.

Conduct outreach to identify youth in need of Redfield Academy services.

Participate in truancy-related efforts such as sweeps, and School Attendance Review Boards (SARB).

Maintain accurate documentation and case notes in designated case management databases, ensuring confidentiality and compliance with agency standards.

Prepare and submit monthly reports, case summaries, and eligibility documentation for program compliance.

Provide transportation for youth and families when needed.

Participate in in-service training and staff development opportunities.

Participate in program-related activities, including staff and parent meetings, and Redfield events (e.g., field trips, awards ceremonies).

Perform additional duties as assigned to support the agency's mission and contribute to the overall success of its programs.

Develops and maintains individualized service plans aligned with education, workforce, and long-term goals

Functions as a systems navigator across education, workforce, probation, and community service providers

Communicates effectively and collaborates within multidisciplinary teams, including instructors, job developers, and leadership

Maintains accurate documentation, compliance, and data integrity aligned with program and grant requirements across multiple data systems.
