Definition

The Parents as Teachers (PAT) Case Manager develop partnerships with families who are pregnant or have children ages six and under. PAT is an evidence-based program that enables the parents to work directly with their child on developmentally appropriate, skill building activities. The PAT Case Manager helps parents understand early childhood development concepts and terminology that increases the parent's ability to observe and understand their child's learning process.

The PAT Case Manager provides "wraparound services" to families participating in the PAT program and is responsible for providing information/referrals, case management, outreach, support services and direct services. The PAT Case Manager will also provide community, networking, and family engagement activities monthly at the Group Connections. This position requires an understanding of early childhood education and indicators of quality care, school readiness, and an understanding of strengths-based case management.

This position will report to the PAT Program Coordinator.

Duties
  • Conducts client intakes for PAT participants.
  • Responsible for establishing eligibility for families participating in PAT Wraparound Services.
  • Enters, updates, and monitors information in the case management database and Visit Tracker.
  • Provides parent support and ongoing education of child development for PAT parents as required.
  • Participates or attends PAT program activities, including staff and parent meetings.
  • Assist families with transition to kindergarten, including providing kindergarten registration information and coordinating kindergarten visitations.
  • Participation in weekly or bi-weekly Reflective Supervision with the PAT Coordinator.
  • Participation in the planning and orchestration of group meetings, which includes site connection, planning, connecting the curriculum and the provision of an enrichment activity.
  • Participation in in-service training and other staff development activities.
  • Keeping the PAT Coordinator abreast of child/family progress and familial issues and concerns as they arise.
  • Timely completion and submission of required forms, screenings and other documentation as required by the evidence-based programming
  • Participation in all sponsored PAT activities (e.g., Group Connections, graduations, awards ceremonies, etc.);
  • Assisting the Coordinator in maintaining an efficient record keeping system for participating families.
  • Achieve an understanding of agency mission, strategic focus areas and overall goals and objectives. Contribute to the success of these areas.
  • Read, understand, and apply all agency and department policy and procedures.
  • Have a complete understanding of the agency's programs and community resources to provide information and referrals to families through case management services;
  • Conducts follow-up activities with families.
  • Facilitative case management and carry a caseload of up to 15 families.
  • Conducts outreach at specified sites to identify families in need of services.
  • Submits monthly reports as directed.
  • Perform other duties as assigned
Minimum Qualifications Knowledge of
  • Appropriate interactions with children and families.
  • Community agencies and resources, as well as an understanding of the problems facing youth and families.
  • Positive parenting concepts and work/life supports for families.
  • Family and youth relationship issues.
  • Sound decision-making, crisis management techniques, be self-motivated and a team player.
  • Case Management skills with an emphasis on wraparound and facilitating services.
  • Computer applications and the desire to learn new programs.
