$145.00 hr – $145.00 hr (base hourly rate for each in‑home health evaluation completed).

The number of in creator health evaluations completed per day may vary. Pay depends on experience, education and geography. The position is eligible for additional reimbursement for completed diagnostic and preventive services.

• Be flexible to travel locally and within licensed states as needed.
• Conduct virtual visits as required.
• Provide all Signify Health services, including diagnostic and preventive services such as spirometry, diabetic eye exams, bone density screening, and others.

As an in‑home clinician, your primary workplace is the private residence of health‑plan members. Conditions may vary, including pets, second‑hand smoke or other allergens. Candidates must be able to perform duties safely and comfortably in these settings.

Many of the people I see are shocked that a physician comes to their home for a one‑on‑one visit. When I sit down and have an unhurried conversation about their health, they are impressed, happy and understand that their health plan cares about them.

The primary care physician gets a snapshot of a patient in an office setting, but with Signify Health we see the environmentｯﾁｮｲ Their home, the state of disrepair, and the person’s ability to function comfortably. We can make referrals to the health plan, encourage participation of loved ones, and ensure деген PCPs are aware of their health and overall quality of life.
