Physical Therapist (PT) Lacamas Creek Post Acute Full-Time: $40-55 per hour PRN: $60-65 per hour
Lacamas Creek Post Acute is looking for a compassionate and motivated Physical Therapist (PT) to join our rehabilitation team. We're looking for someone who loves helping patients regain strength, mobility, and independence while being part of a supportive and collaborative team. What You'll Do- Evaluate patients and develop individualized plans of care based on their needs and goals
- Provide skilled physical therapy services to help patients improve strength, mobility, balance, and overall function
- Monitor patient progress and adjust treatment plans as appropriate
- Educate patients, families, and caregivers on exercises, mobility, and strategies to support continued progress
- Accurately document evaluations, treatments, progress, and outcomes
- Collaborate with Physical Therapist Assistants, nursing, and other members of the interdisciplinary care team
- Provide guidance and supervision to Physical Therapist Assistants as appropriate
- Help create a positive, encouraging rehabilitation experience for every patient
- Graduate of an accredited Physical Therapy program
- Current Physical Therapist license or eligibility for licensure
- Strong clinical, communication, and interpersonal skills

- Full-Time Pay: $40-55 per hour
