House Calls Advanced Practice Clinician (NP/PA)

Optum Home & Community Care is seeking a Nurse Practitioner (NP) or Physician Assistant (PA) to join its House Calls team. This role focuses on conducting annual in-home health assessments for Medicare Advantage and other health plan members.

What You'll Do

• Conduct comprehensive in-home health assessments
• Review medical history and medications
• Perform physical exams, screenings, and point-of-care testing as appropriate
• Identify and document diagnoses
• Communicate findings and care gaps to primary care providers
• Educate patients on conditions, medications, and health management
• Address social determinants of health and connect members with resources
• Recognize and respond to urgent or emergent situations

Required Qualifications

• NP: ANCC or AANP certification (Family, Adult, Geriatric, or AGPCNP)
• PA: NCCPA certification
• Active state licensure (or ability to obtain before start)
• Valid driver's license and reliable transportation
• Ability to travel to patients' homes and navigate various home environments

Preferred Qualifications

• At least 1 year of clinical experience (family practice, geriatrics, home health, etc.)
• Experience working with Medicare or geriatric populations
• Comfortable using EMR systems and technology
• Ability to carry equipment up to 30 lbs.
