Care Manager

Elder Tree Care Management partners with older adults and their families to navigate aging with clarity, dignity, and confidence. We support clients through complex medical, cognitive, and life transitions by coordinating care, advocating across systems, and serving as a trusted guide during some of life’s most difficult moments.

Care management is a holistic, relationship-driven service that helps older adults and their families manage health, safety, and quality-of-life needs as they age.

• Assess medical, cognitive, functional, and psychosocial needs
• Create and oversee personalized care plans
• Coordinate with physicians, therapists, facilities, and home care providers
• Advocate for clients and families across healthcare and community systems
• Provide ongoing guidance, monitoring, and problem-solving as needs evolve
• Work across the metro DC area to provide clinical advocacy and psychosocial support for our clients

As a Care Manager, you will work directly with older adults and their families to assess needs, develop care plans, and provide ongoing oversight and advocacy. This role blends clinical judgment, care coordination, and relationship-based work.

You’ll be supported by a collaborative, mission-driven team while maintaining ownership over your caseload. Our hours of operation are Monday - Friday, 9 am - 6 pm EST, with the expectation of being on a rotating on call schedule for periodic weekends. Individuals must drive a car, have a good driving record, and be able to see clients across the DMV (mileage & tolls reimbursed)

• Recreational Therapist / Associate Recreational Therapist
• Gerontologist
• Music Therapist

Prior care management experience is helpful but not required — we provide training and mentorship.

• Conduct in-home and facility-based assessments
• Develop and manage individualized care plans
• Coordinate medical and community services
• Communicate regularly with families and providers
• Monitor changes in condition and adjust plans accordingly
• Document clearly and professionally
• Serve as a calm, trusted presence during crises and transitions

• Licensed or credentialed professional in a relevant discipline
• Strong clinical judgment and critical-thinking skills
• Excellent communication and interpersonal skills
• Comfort working independently in the field
