Hospice​/Palliative Nurse Practitioner or APRN

We are seeking a skilled and compassionate Nurse Practitioner (NP) to join our hospice team. The NP will provide medical care to hospice patients in collaboration with the interdisciplinary team, focusing on comfort, dignity, and quality of life.

• Conduct comprehensive assessments of hospice patients
• Collaborate with physicians, nurses, social workers, and chaplains
• Develop and manage individualized care plans
• Provide symptom management and palliative care interventions
• Educate patients and families about care goals and options
• Ensure documentation meets hospice and regulatory standards
• Participate in interdisciplinary team meetings and care planning

• Board certification as an NP (AGNP, FNP, or equivalent)
• Minimum of [1–2+] years of hospice, palliative care, or geriatric experience preferred
• Strong communication and clinical decision‑making skills
• Empathetic, patient‑focused, and team‑oriented
