Reston, VA
Pay From: $40 per hour
- BSN required.
- 3+ years of nursing experience, preferably in community health, public health, primary care, ambulatory care, or a similar setting.
- Previous supervisory, charge nurse, team lead, or clinical leadership experience preferred.
- Strong knowledge of patient care coordination.
- Excellent communication, organization, and leadership skills.
- Ability to work effectively with patients from diverse backgrounds and with multidisciplinary teams.
- Strong clinical judgment and problem-solving abilities.
- Proficiency with electronic medical records and standard computer applications.
- Must be a Registered Nurse licensed in the state of Virginia
- Must enjoy working in Community Health
- Must have BLS certification
- Must be able to travel between multiple locations
- Team player, compassionate, attention to detail
- Supervise, lead, and support RNs, LPNs, and other clinical staff.
- Provide clinical guidance, education, and mentorship to nursing team members.
- Oversee daily nursing operations and help ensure appropriate staffing and patient coverage.
- Coordinate care for patients with complex medical and social needs.
- Conduct patient assessments and assist with the development and implementation of individualized care plans.
- Monitor patient outcomes and identify opportunities to improve quality of care.
- Collaborate with physicians, advanced practice providers, social workers, case managers, community organizations, and other healthcare professionals.
- Help connect patients with appropriate community resources, healthcare services, and social support programs.
- Monitor compliance with nursing standards, organizational policies, and applicable regulations.
- Participate in quality improvement initiatives, audits, and performance improvement activities.
- Assist with onboarding, training, competency assessments, and performance management of nursing staff.
- Address clinical concerns and provide appropriate follow-up and escalation when needed.
- Maintain accurate and timely patient documentation.
- Promote patient education, preventative care, wellness, and chronic disease management.
- Participate in outreach and community health initiatives as needed.
- Serve as a clinical resource for staff and help resolve patient-care issues.
- Perform other duties as assigned.
