* Position Summary:

** Full-time RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location. Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care.  Current MA RN License required with minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred.

A minimum of one year experience working with a frail or elderly population. Prior experience in interdisciplinary team setting a plus.

Schedule:

Monday  - Friday, 8:00 AM - 5:00 PM, and will include weekend/holiday/on-call rotation.

* Essential Duties and Responsibilities:

*** Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
* Prepares and presents a patient for rapid provider review and treatment.
* Arranges for or directly implements care plan interventions and testing, treatment, or other provider orders for a panel of participants.
* Oversees delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed.
* Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider.
* Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.
** Qualifications and Requirements:
*** Baccalaureate degree in nursing preferred.
* Current Massachusetts license in nursing.
* Current BLS certification required.
* Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
* A minimum of one year experience working with a frail or elderly population.
** Skills/Abilities:
*** Basic physical assessment and triage skills.
* Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats.
* Ability to communicate effectively with professional and non-professional staff, outside providers, participants and families in person and by phone. Bilingual a plus.
* Excellent customer service skills.
* Flexibility, initiative, creativity and excellent organizational skills.
* Basic computer skills including experience with word processing and email.
* Must be able to efficiently and effectively navigate windows-based programs and Neighbor Health’s EMR.
** Pay Range:
*** Starting at $40.00 up to $60.00 depending on experience
