Department: PACE Primary Care - Nursing

All Locations: 10 Garofalo Street - Revere PACE

Position Summary

Full-time RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location. Responsible for day-to-day nursing assessment and for the delivery and documentation of quality patient care. Current MA RN License required with minimum of 1 year of experience in ambulatory, home health, hospital or sub-acute setting preferred. A minimum of one year experience working with a frail or elderly population.

Prior experience in interdisciplinary team setting a plus.

Schedule:

Monday - Friday, 8:00 AM - 5:00 PM, and will include weekend/holiday/on-call rotation.

Essential Duties and Responsibilities
  • Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
  • Prepares and presents a patient for rapid provider review and treatment.
  • Arranges for or directly implements care plan interventions and testing, treatment, or other provider orders for a panel of participants.
  • Oversees delegated tasks to ensure completion, and monitor outcomes, and make adjustments to care plan as needed.
  • Safely and effectively administers medications, tests and treatments as ordered. Manages the refills of all patient medications with appropriate authorization by the provider.
  • Teaches patients and families health care regimens. Explains and provides information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.
Qualifications and Requirements
  • Baccalaureate degree in nursing preferred.
  • Current Massachusetts license in nursing.
  • Current BLS certification required.
  • Minimum of 2 years experience in ambulatory, home health, hospital or sub-acute setting preferred, equivalent training or related experience considered.
  • A minimum of one year experience working with a frail or elderly population.
Skills/Abilities
  • Basic physical assessment and triage skills.
  • Ability to communicate assessment and triage findings efficiently, effectively and accurately in verbal and written formats.
  • Ability to communicate effectively with professional and non-professional staff, outside providers, participants and families in person and by phone. Bilingual a plus.
  • Excellent customer service skills.
  • Flexibility, initiative, creativity and excellent organizational skills.
  • Basic computer skills including experience with word processing and email.
  • Must be able to efficiently and effectively navigate windows-based programs and Neighbor Health's EMR.
Pay Range

Starting at $40.00 up to $60.00 depending on experience
