Job Summary

We are seeking a Registered Nurse or Licensed Practical Nurse to join our growing team at Jethro Health System. You will provide high-quality nursing care to patients in their homes, ensuring their needs are met with compassion and professionalism.

• Assess the patient’s condition during every visit and document observations.
• Perform tasks including vital signs and medication review.
• Administer medication as prescribed by the patient’s physicians.
• Perform wound care and assess progress of healing.
• Educate patients and families on proper home health care strategies.
• Coordinate with physicians and other care team members.
• Recommend devices/tools that might improve the patient’s quality of life.
• Listen to the patient and respond to concerns or requests.
• Complete all clinical documentation accurately and in a timely manner.

• Active Licensed Practical Nurse (LPN) license in Massachusetts.
• 1–2 years of clinical experience (home care experience preferred).
• Strong communication and organizational skills.
• Ability to work independently in the field.
• Familiarity with EMR systems.
• Reliable transportation.
