Position Summary
Full‑time Nurse Practitioner position available immediately to provide primary care services to frail participants of the Neighborhood PACE of East Boston program. The role requires independent primary care delivery to a panel of participants in multiple geriatric settings, including home care, nursing homes, and a day center, in partnership with an interdisciplinary team.
Essential Duties & Responsibilities- Obtain comprehensive and problem‑focused health history
- Perform comprehensive and problem‑focused physical exam
- Order and interpret screening and diagnostic tests
- Demonstrate critical thinking and diagnostic reasoning skills in clinical decision making
- Communicate with patients regarding clinical findings
- Initiate appropriate and timely consultations with NP, PA, or MD
- MA Nurse Practitioner license
- BLS certification
- 1 year of relevant experience, preferably in a community health setting and with frail adults (significant experience as a practitioner for frail adults preferred)
- Adult or Family Medicine certification considered
From $135,000 to $164,564 based on year of experience at the licensure level.
