RN Primary Nurse PACE

Position Summary

Full‑time RN position available in our PACE Center, which provides integrated medical, rehabilitation, case management, and adult day health services all in one location. Responsible for day‑to‑day nursing assessment and for the delivery and documentation of quality patient care.

• Assess a patient through obtaining history, performing appropriate tests, making or collecting observations from the participant and caregivers, and consulting with other members of the IDT.
• Prepare and present a patient for rapid provider review and treatment.
• Arrange for or directly implement care plan interventions and testing, treatment, or other provider orders for a panel of participants.
• Oversee delegated tasks to ensure completion, monitor outcomes, and make adjustments to the care plan as needed.
• Safely and effectively administer medications, tests and treatments as ordered.
• Manage the refills of all patient medications with appropriate authorization by the provider.
• Teach patients and families healthcare regimens.
• Explain and provide information regarding disease and wellness, tests, results, medications, diet, exercise, and other aspects of the plan of care to patients.

• Baccalaureate degree in nursing preferred.
• Current Massachusetts license in nursing.
• Current BLS certification required.
• Minimum 2 years experience in ambulatory, home health, hospital or sub‑acute setting preferred; equivalent training or related experience considered.
• Minimum one year experience working with a frail or elderly population.

• Basic physical assessment and triage skills.
• Ability to communicate assessment and triage findings efficiently, effectively, and accurately in verbal and written formats.
• Ability to communicate effectively with professional and non‑professional staff, outside providers, participants and families in person and by phone.
• Bilingual a plus.
• Excellent customer service skills.
• Flexibility, initiative, creativity and excellent organizational skills.
• Basic computer skills including experience with word processing and email.
• Must be able to efficiently and effectively navigate windows‑based programs and Neighbor Health’s EMR.

Starting at $40.00 up to $60.00 depending on experience. Includes a $10,000 sign‑on bonus for qualified candidates.
