Health Innovations is seeking a Harm Reduction Nurse Case Manager to join the Health MOVES Mobile Team to provide compassionate on‑demand public health care including HIV/STI/Hepatitis C testing, linkage to care, PrEP, overdose prevention in Revere/Chelsea. The ideal candidate is a compassionate and self‑motivated individual who embraces the philosophy of harm reduction and is committed to delivering high quality nursing care in a low‑threshold, harm reduction community environment.

Under the direct supervision of the Clinical Director and general supervision of the Health MOVES Program Manager, the Harm Reduction Nurse Case Manager conducts prevention education and counseling, performs brief and comprehensive sexual, substance‑use/mental‑health risk assessments, provides preventive health screenings (HIV, STI, VHP), provides SSP/overdose prevention services, and administers immunizations to underserved and at‑risk populations. The incumbent also delivers test results, coordinates referrals for patients testing negative or positive, and assists with MOUD assessment and telehealth navigation.

• Direct nursing care to ensure all patient needs are met according to agency policies and Department of Public Health regulations.
• Perform primary care nursing services, including HIV/STI/VHP prevention education, risk assessment, screening, and vaccine administration.
• Ensure sedated patients are triaged and safely monitored when sedation is required.
• Deliver HIV/STI/VHP results to patients or coordinate delivery with outside providers.
• Coordinate supported referrals and linkage to care for at‑risk negative and positive patients.
• Track completion of supported referrals and document status/progress.
• Follow up with patients 2‑4 weeks post‑nurse visit for prevention/harm‑reduction plans and vaccination reminders.
• Provide short‑term case management and health navigation during transition to care appointments.
• Participate as a member of the interdisciplinary care team.
• Identify and care‑plan high‑risk/complex patients.
• Distribute condoms, lube, safer injection equipment, brochures, and other culturally appropriate education materials.
• Administer Narcan and provide overdose prevention education and enrollment.
• Serve as first responder in medical emergencies on site.
• Engage patients with substance use disorder consistently with harm‑reduction principles.
• Provide basic wound care and medical assessment.
• Maintain electronic medical records (EMR) and required funder data‑collection forms.
• Participate in ongoing clinical quality improvement.
• Supervise and mentor RN students or volunteers during clinic sessions.
• Attend meetings, conferences, and trainings to expand knowledge.
• Complete paperwork, documentation, and reporting on time.
• Establish and maintain good rapport with staff, patients, and partners.
• Execute duties to reflect safety standards, using universal precautions.
• Perform other related duties as needed or as directed.

• Maintain direct communication with Clinical Director and Program Director/Staff.
• Maintain a professional demeanor with participants, colleagues, and supervisors.
• Utilize motivational interviewing techniques.
• Ensure accurate and complete clinical record documentation.
• Attend program staff meetings and clinical supervision.

• Comfortable with street‑based practice and outreach to mobile sites.
• Responsibilities may change to meet program needs at the discretion of the Clinical Director.

Educational Requirements

• Bachelor of Science in Nursing (BSN) from an accredited university.

Licensure & Certifications

• Registered Nurse (RN) by the Massachusetts Board of Nurse Examiners.
• ACRN certification preferred.
• MDPH OHA certifications preferred:
  Fundamentals of HIV/AIDS, Behavioral Risk Assessment, HIV.
