Job Overview

Elite is seeking a compassionate and dedicated Physical Therapist to join our team in providing exceptional home health and hospice care. Responsibilities
  • Assume responsibility for the ongoing interdisciplinary assessment and development of individualized plans of care in partnership with patients and caregivers.
  • Provide counseling and education to patients, caregivers, and families.
  • Prepare clinical notes and progress summaries based on attainment of goals.
  • Communicate with physicians and other health‑care practitioners related to the current home health plan.
  • Assess therapeutic and functional status, including muscle strength, mobility, gait, and ROM, and evaluate the home environment.
  • Direct patient physical‑therapy treatment and determine priority needs.
  • Initiate physical‑therapy programs and instruct personnel or family/caregiver members in treatment phases.
  • Participate in case conferences, discharge planning, and the peer‑consultation process.
  • Provide in‑service education for nursing‑organization personnel as needed.
  • Supervise Physical Therapy Assistants in accordance with organization policy and state regulations.
Qualifications
  • Licensed to practice as a physical therapist in the State of Idaho and the State of Washington.
  • Passed a state‑approved examination for physical therapists and holds a bachelor’s or master’s degree in physical therapy accredited by CAPTE (or successor).
  • Possesses and maintains current CPR certification.
  • Must be a licensed driver with a reliable, insured automobile in good working order.
