Elite Home Health & Hospice is a Medicare‑certified home health and hospice agency that serves Asotin & Garfield counties in Washington and Nez Perce County in Idaho. Our team of professionals is dedicated to providing life‑changing service in your home under the direction of your physician.

• Complete an initial, comprehensive, and ongoing assessment of patients and families to determine home health or hospice needs.
• Provide respectful, compassionate care in patients’ homes.
• Develop patient‑focused care plans that establish goals based on nursing diagnoses and incorporate rehabilitative nursing actions.
• Collaborate with interdisciplinary team members.

• Active nursing certification (state‑licensed).
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong communication skills and a team‑oriented mindset.
• Ability to work independently in home settings.
